Healthy Ageing
Our supported living assistance is designed for older adults who require structured, clinical exercise support to maintain independence and function safely in their home or community environment.
We work with individuals managing chronic health conditions, reduced mobility, or increased falls risk, delivering personalised exercise physiology programs that focus on strength, balance, and functional capacity. Interventions are tailored to support activities of daily living such as walking, transfers, and general mobility, with a strong emphasis on safety and progression at an appropriate pace.
We regularly support referrals from GPs, families, and allied health professionals, providing clear communication and consistent follow-up. Our approach prioritises long-term outcomes—helping individuals maintain independence, reduce falls risk, and improve confidence in their day-to-day lives.

How Referrals Work
Referring to Move You Exercise Physiology is simple and straightforward. We aim to make the process easy for referrers, participants, and families.
1. Get in touch
Referrals can be made by contacting us directly via phone, email, or through the website contact form (below). You can also include any relevant clinical information to support the referral.
2. Initial assessment
We arrange an initial assessment to understand the individual’s needs, goals, and current capacity. This allows us to develop a personalised, evidence-based exercise plan.
3. Individualised program
A tailored program is implemented, focusing on improving strength, balance, mobility, and overall function, with progression based on the individual’s response.
4. Ongoing communication and reporting
We provide clear communication and regular updates where required, including reports to support coordinators, GPs, and allied health teams.
